Can someone point me to information on the process of taking advantage of the PDGA membership discount for affiliate club members?

I understand that the club has to collect the information and money, then submit them in a block of 10 or more.
I cannot seem to find an on line form for filling local members' names. Is it something they send to you after you register your club?

Here's the PDGA page on Affiliate Clubs. First your club has to become an Affiliate for $25. Then, each person gets $5 of of their renewal or new membership. What I believe happens is you collect filled out printed renewal forms from at least 10 people and send in the checks for the total. But contact Cliff as noted on this page for more info.

This benefit of the ACP has been streamlined. No longer need to submit bundles of 10 and if your club is one of those that extends the $5 discount as a benefit to your members - you can just send them online to renew with a credit card - easy admin for the club.

Nov 1st 2007 through Aug 31st 2008 and will include only 2008 ACP handled PDGA memberships that are submitted by active 2008 PDGA Affiliate Clubs
